We are happy to be offering retiree dental coverage through Delta Dental. Please review the description of benefits and costs before making your decision. Coverage will be provided at 100% for preventive treatment, 75% for basic care and 50% for major services. The annual maximum is $1,000. You can roll over a portion of your unused spending to increase your maximum benefit limit next year and beyond. Please review the Delta Dental Rollover Max brochure for full details.

Important Notes:

1. To elect coverage for you and your dependent(s) you must elect a ‘family’ plan.
2. The monthly cost for individual coverage is $66.77. The monthly cost for family coverage is $180.24.
3. You will be direct-billed for the dental coverage by CROSBY BENEFITS SYSTEMS, INC.
4. Once enrolled in the dental plan you will receive a separate identification card from Delta Dental.
